What to do next

In order. Whoever is covering the desk can follow these.

  1. Find the revalidation or recredentialing due date on the payer's portal. Medicare posts it about 7 months ahead.
  2. Make sure the CAQH attestation is current, because commercial payers read the profile.
  3. Submit the application well before the due date. Medicare gives no extensions for a missed revalidation.
  4. When the payer confirms, record the new revalidation date here and verify it on the portal.

Renewal workflow

2. Renewal came back

The new term has to run later than the current one, and not longer than 120 months. Attach a copy of the renewed credential or link to it, so anyone checking can open it.

3. Verified at source

Confirmed with the issuer, not from the provider's copy. A renewal recorded early can be verified as soon as the issuer shows it, before the new term starts.
