  1. Ask the medical staff office for the reappointment packet.
  2. Complete it with the provider and gather what it asks for, such as case logs and peer references.
  3. Send it in time for the credentials committee, which usually meets monthly.
  4. Record the new appointment period here when the approval letter arrives.

The new term has to run later than the current one, and not longer than 48 months. Attach a copy of the renewed credential or link to it, so anyone checking can open it.

Confirmed with the issuer, not from the provider's copy. A renewal recorded early can be verified as soon as the issuer shows it, before the new term starts.
