  1. Ask the carrier for the renewal application a few weeks before the policy ends.
  2. Send the completed application with the claims history the carrier asks for.
  3. When the carrier binds the new term, record it here and file the certificate of insurance.
  4. Send the new certificate to every payer and facility that asks for proof of coverage.
