State licenses, DEA and state controlled substance registrations, board certification, malpractice, CAQH attestation (the shared profile payers read), payer enrollment and revalidation, hospital privileges, CPR, and the monthly exclusion check.

It is for the credentialing coordinator who keeps all of that current, and the practice manager who needs to know nobody is working on a lapsed license. It needs no patient data at all.
